Customized Healthcare for Men

Our Lady of the Lake Physician Group Men's Health Clinic is a membership-based center designed by men for men with a goal of making it easier for them to take control of their health. It is recommended that when joining patients are assessed with an Executive Wellness Exam. This exam is supplementary to the membership but is discounted for members and is the most extensive exam we offer. Members of the Lake Men's Health will experience unmatched service in three major areas.

Access

When you have an issue, we want to be able to address it as quickly as possible. We offer same- or next-business-day appointments for any medical concern you may have to help you return to a good state of health as soon as possible.

Choice

We want you to be able to access care however it makes sense for you. While periodic visits for chronic illnesses are important, in most instances we can easily and confidently handle simple medical issues, refills and medical questions over the phone, by secure email or even over a video chat.

Efficiency

We will do everything we can to ensure there is little to no waiting in the center once you arrive. For a typical visit, you should be in and out in less than 30 minutes. All blood work is conveniently drawn in the center by our nurses.

  • Junior Membership

    This membership is for men between 16 and 30 years of age.

    Includes:

    • Annual Health Review
    • Direct access to care providers during business hours via phone, text, email or video visit
    • Discounts on additional imaging and lab tests
    • Extended time with the doctor/nurse practitioner
    • Same-day/next-day appointments and drop-ins

    Cost is $1,000 per year in one lump sum or $1,104 per year, payable in 12 monthly installments of $92.

  • Men’s Health Membership

    This membership is for men over 30.

    Includes:

    • Annual Health Review
    • Direct access to care providers during business hours via phone, text, email or video visit
    • Discounts on additional imaging and lab tests
    • Extended time with the doctor/nurse practitioner
    • Same-day/next-day appointments and drop-ins

    Cost is $2,000 per year in one lump sum or $2,100 per year, payable in 12 monthly installments of $175.

  • Platinum Membership

    This membership is available for up to 50 men and women.

    Includes:

    • Executive wellness exam
    • Center follow-up care
    • Unlimited access to your coordinator
    • After-hours consultations
    • Annual consultation with nutritionists/personal trainer
    • Online communication through MyChart
    • Same-day/next-day appointments and drop-ins
    • Urgent and routine referrals to specialists
    • Expert recommendations for specialty care and direct consultations between your physician and specialist
    • Insurance filing handled through Platinum coordinator
    • Emergency room/after-hours triage coordination
    • Online visits and online prescription refills
    • Travel immunizations
    • Pre-operative clearance for surgery
    • Healthcare reminders
    • Assistance with additional procedures, such as CT scans and Botox injections
    • A spouse or a significant other is eligible to join the membership plan at an additional cost.

    Cost is $12,000 per year in one lump sum. You may purchase a Platinum Membership for your spouse for an additional $8,000.

Men's Health Services

As a member of the Lake Men's Health Center, you have access to two main types of visits: the Annual Health Review and the Acute Care Visit.

Annual Health Review

This is a comprehensive review that allows us to address health issues, medications, and immunizations, and provide healthcare recommendations to help prevent disease. 

The AHR includes a thorough physical exam, review of all blood work, imaging if indicated, and advice regarding any threats to your health. A thorough laboratory panel is included in your membership which is more robust than the annual labs typically covered on most insurance plans. You also have the option to upgrade to an Executive Wellness Exam which includes more robust imaging options. 

Acute Care Visit

In the event you experience a minor illness or injury, we will work to resolve it as quickly as possible. You can reach us via the following options.

Frequently Asked Questions About Our Services

  • If I don’t sign up now, can I sign up later?

    Certainly. However, membership will be closed once capacity is reached. At that time, we will establish a waiting list and take new members on a first-come, first-served basis. 

  • How will this affect my insurance?

    Since you will require fewer trips to our office, it may be advantageous to consider increasing your annual deductible, thereby reducing your monthly premiums to help offset your membership fee. Additionally, Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA) can be used to offset the cost of any “qualified expense” not reimbursed by your insurance company. If your medical issue requires a visit to our office, we will bill your insurance company for the visit. You will be responsible for any coinsurance or copay required by your insurance company. 

  • Will my insurance cover my annual fee?

    Insurance companies will not cover your annual membership fee.

  • Is there a limit to the number of members?

    Since the goal is to have more time with our patients during each visit, there is a finite number of members, but we are currently accepting new patients. 

  • Is there an option for a family membership?

    While we currently do not offer family memberships, we have options that cater to other members within your family.  

    Our Junior Membership is for males between the ages of 16 and 30, and costs $1,000 annually. 

    If you are a Platinum member, you can add your spouse to your membership for an additional $8,000 annually. 

  • Can my wife join?

    While our Men's Health memberships are designed for men only, Platinum members do have the ability to add their spouse to their membership pan for an additional $8,000 annually. 

  • Can my son join?

    Yes, we offer a Junior Membership for males between the ages of 16 to 30 years old that costs $1,000 annually. 

  • What is the minimum age?

    Men ages 16 and older are eligible. 

  • I have been getting an Executive Wellness Exam every year. How will those work with the membership?

    Your membership includes a discounted rate for the Executive Wellness Exam which is supplementary to the membership itself. 

  • Which doctor will I see if I come to the center?

    As a patient of Lake Men's Health, you will see either Dr. Curtis Chastain, Dr. Tyler Boudreaux, or Dr. Oscar Shoenfelt depending on their availability and your type of visit.

  • Can I text medical questions?

    You can request an appointment or ask a simple non-secure question through text message, and we will respond in a timely manner. As texting is not secure, we do not recommend sending detailed medical questions by text.

  • Can I cancel my membership?

    Absolutely. If you paid a lump sum for your enrollment and have not had your Annual Health Review, we will gladly refund a prorated amount of your membership fee. 

  • Do I get your cell phone number?

    We have a designated office cell phone that you will have unrestricted access to call or text during business hours. 

  • How do I access your office after hours?

    If a situation arises after hours, please call the office and you will be directed to our answering service. They will assess your issue and contact one of us if necessary. 

  • What if I need to go to the hospital?

    If you require hospitalization, you will be admitted by one of our physician colleagues. He/she will manage your care during your entire hospital stay and will be in direct and constant communication with us regarding your care. 

  • What if I need emergency room services?

    Our physician colleagues in the emergency room will see you and contact us if necessary. 

  • Will you help me with access to specialists?

    If your medical issue requires a referral to a specialist, we will make a recommendation for you and help facilitate your appointment. However, since you know your schedule best, it is optimal for you to make your appointment. 

  • How much is the Men’s Health membership?

    The annual membership fee has two options: a $2,000 lump sum annual fee or monthly installments of $175. The monthly installments will be collected via automatic payments from your debit card, credit card, or checking account. If your company provides this service for you, please provide the contact information, and we will invoice your company.
